Applicants should assemble items 1-10 and submit these documents in one envelope. Use the following checklist as a guide. Please submit the signed checklist with your completed application. The deadline to submit your completed application with all required documents is April 28, 2006. After the deadline the applications with missing items will not be considered for admission.

_____3. Official NLN exam scores, if exam is not taken at LICHSON. Please call (800)669-9656 to request your official scores to be sent directly to us. The LICHSON program code is 21262. For registration for NLN exam at LICHSON, please refer to the attached NLN Examination Registration Information Form.

_____4. Sealed official transcript of your high school record. All foreign high school transcripts must be evaluated by World Education Services (phone # (212) 966-6311).

_____5. General Equivalency Diploma transcript. Please request the official scores to be sent directly to LICHSON (For GED taken in New York state phone # (518) 474-5906).

_____6. Sealed official transcripts from the Registrar’s Office of each college and post-secondary institution you have attended. All foreign college transcripts must be evaluated by World Education Services (phone # (212) 966-6311).

_____7. Personal Essay 250 words. Please refer to the application for essay topics.

_____8. Two letters of recommendation (academic and/or employment) on official letterhead stationary.

_____9. Photocopies of your proof of citizenship or permanent alien resident card. No applicants with visas are accepted.
